Praise the Lord for the rain!
Two bad nights I was in charge. Afternoon one, I get to work, find out I am in charge, okay, but all junior staff again?? So i have two RN's I am wary about and it is not until I have allocated that I really realise what I have done. As it was I had 10 patients with an EN. The EN's I am working with keep playing with the IV's when they shouldn't be, its irritating, both nights I have found antibiotics half through and IV's stopped and stuff.
So I allocated one nurse a lady who was not very well at all, she has a central veinous line (CVC), these lines are ones that go into the big veins in your neck or just below your shoulder bone, so they are close to your heart and it is important that everything is done aseptically with them and that lots of care is taken. The lines are what is called heparin locked, they put 5mls of heparin in them which is an anticoagulant and it stops them from clagging off. If the line is being used we always have some sort of fluids running through them because they can get blocked easily. The RN I allocated to this woman is supposedly not a new grad and supposedly knows what she is doing, but unfrotunately she doesn't really. She tried to open one of this womens heplocked lines so she could flush it... which is something we never do... fortunately someone stopped her. She didn't put up a new bag of TPN up on this woman, I asked her at the START of the shift if she was okay with the TPN and changing it and she said yes. If she had ANY doubts all she had to do was ask... but she didn't and the TPN didn't get changed and we copped the flack for it the next day. She was giving all sorts of medications late.
Then when one of the other guys left I found out he had forgotten to give a heap of medications also. BLEH... I CANNOT do my own work and be expected to be looking over their shoulders all the time.. it is not humanly possible.
The second night, I had 3 ladies who were all post operative after large operations with PCA's hourly obs, it made for an excruciatingly busy night.
I got all of three hours sleep last night cause I got home late and then I couldn't switch off my damn brain. I go over things over and over, and realise the things I have forgotten. Its full of crap.
So today it rained, and last night... it rained all day, so we got flooded in, can't get out under the bridge, I had to wade through the freezing water in the rain to get in. Yaaaaay for the raiiiiin!
Taking tomorrow off.
