Not so good a day today, we had a guy who was okay, NFR (not for resusitation) but okay at the start of the day. He was looking well this morning, we even got him out of bed for a few moments, but he was very tired and we put him back to bed fairly quickly. So about 11:30 or so I walk in and hear him very rattly in his chest so I thought I would go back and check him out and make sure he was okay, but I had 9 other patients that I was trying to keep an eye on as well. So he went off to xray on his bed just after I had been in there, he was at xray for a long while. He returned at about 12:30 and was really labouring for breath, the xray attendant told me he needed a bottle, I checked and he had pooped all in his bed, he was grey and he was peripherally shut down (as in the blood was flowing mostly away from his limbs and into his main circulation to keep his heart pumping) which means he will probably die soon if nothing was done. So I talked him over with a couple of people and we called an emergency call on him. So the doctors come, and I want them to give the man some lasix cause he had fluid in his lungs, and he is NFR so they should make him comfortable... but nope, they want to rescuisitate him firstly, then finally they decided to get respiratory to come see him. So the respiratory team see him and we get him written up for regular morphine if he needs it, we clean him up and sit him up but I can tell by his breathing that he is close to the end. i went and grabbed some lunch because I had not eaten all day, I came back and told the TEN to go eat something, she turned and said he had stopped breathing, We both stood with him as he took his last breaths. Sad, very sad. So then everything was late and I forgot to give out drugs and TPN didn't come and bleh.
